If you are applying Questbridge then we will not need the non-custodial parent profile waiver to be completed.
However, if you do not match and wish to apply regular decision, we will need the non-custodial parent waiver (link).
Due to high volumes, you will need to send the third party attachment with your completed form, be sure to combine all of your information into one PDF file before sending/uploading. This information will be reviewed by our counselors, should they require more information; they will ask upon request. Hope this helps!
