Detail on “Domestic and Eligible Noncitizen Students” and see the instructions under Step 2. You will find a link to the form in the last paragraph of Step 2 along with links on where to submit the form and supporting documentation.
Depending on the situation, supporting documents might include: letters from clergy, letters from school administrators, police reports, letters from healthcare providers, etc.
