Thank you for reaching out. Since there are extenuating circumstances, you can complete the non-custodial parent waiver form (link).
There is an electronic version of the waiver form on IDOC as well through the CSS profile that we will accept.
We also require supporting documentation. This can be a letter of support, usually from your school counselor, stating that there is no contact with the non-custodial parent.
